Produced for the movie in 1995 by Topps and Screamin', package includes the logo plaque (which will lay flat, but also has a hole in the back for wall display) a trading card and a stand for the trading card.

Packaging still has original price tag of 19.95!  Packaging has some storage wear, see pics.  Does not seem to glow in the dark anymore though.
