Sylvester Stallone plays John Rambo, a former lone fighter in a special forces unit who, after returning to civilian life, wants to visit the last friend he believes is still alive from his former Vietnam unit. Not liking Rambo's looks or the way he hitchhikes, the small-town sheriff arrests him to "make him a decent man." He and his acolytes insult and beat Rambo under the cover of their sheriff's uniforms. When they want to cut his hair as well, Rambo breaks free and fights his way into the mountains. In the manhunt that follows, the sheriff is all about principle and John Rambo shows them what a man is capable of who has to fight for his life and thanks to his training he can take on an entire military unit.

John Rambo leads a reclusive life in northern Thailand. On the country's border with Burma, however, the world's longest civil war has been raging for sixty years: the Burmese-Karen conflict. However, Rambo, who lives in the mountains and earns his living by selling poisonous snakes he has caught himself, has long since given up fighting. That doesn't change when paramedics, rebels and refugees pass him through the war-torn country. All this is about to change, however, when a group of missionaries show up and ask "American River Guide" Rambo to lead them up the river to a refugee camp, since anti-personnel mines make the journey there too dangerous. They want to provide medicine, food and Bibles to the persecuted Karen hill tribe who live there. Reluctantly, Rambo finally agrees to guide her and the other helpers. Weeks later, Rambo learns that the missionaries have not returned from the refugee camp, but the helpers have been seen alive, locked in a Burmese army camp. While Rambo's aversion to violence is still pervasive, he knows his help is needed. What follows is the descent into hell on earth.
