Comes with thin case, no cable. I think it's locked to Verizon, I used it as a camera and Wi-Fi only so I can't confirm. See photos for IMEI. Screen is good, Wi-Fi and Bluetooth work, no major issues as far as I'm aware but again I don't know if it can be activated or not
