They will very likely charge a monthly rental for it. It MUST be a card from your provider. Used or new cable cards purchased online, purchased anywhere else, or are from another cable company, will not work. Make sure you can get a cable card (CableCARD) directly from your cable company. Have the cable card in hand and already on your cable company account BEFORE you purchase this TiVo if you want to use it with your cable provider. Returns are not accepted if you purchase this TiVo and then can't get a cable card from your cable company. Not all cable companies use or offer cable cards.
