Simply place the carbon sheet on top of your plaque with the carbon side down. Then, place the design you want to trace over top the carbon paper and trace the lines with the pencil. The pressure from your pencil will cause the carbon on the sheet to transfer your strokes onto the wooden plaque for you to then go over with your wood burning tool.
