This is a 1958 Parkhurst #2 Henri Richard Montreal.

The card is in EX Condition, no crease, id rate it higher but there is some glue residue on the back near the top from being in a scrapbook, otherwise a beautiful card, but im not a professional grader please see pictures and make your own judgment.
