-Pages 27 through 63 have a typed table format meant to be filled in, but, have no writing/are not filled in (except for a brief pencil writing on last page).

Booklet is very fragile and has heavy signs of age/wear:

-the front and back cover have come off and are completely separate from the inner pages.
