From the American Israel Numismatic Association (AINA) is this 2017 Triple Commemorative Medal that commemorates: 

1.) 100th Anniversary of the Balfour Declaration (1917 - 2017)

2.) 50th Anniversary of the reunification of Jerusalem (1967 - 2017)

3.) 50th Anniversary of AINA (1967- 2017)

Design concepts by Mel Wacks and sculpted by Heidi Wastweet, this is # 44 of a limited series of 250 examples produced in antique bronze.

The obverse depicts the United Kingdom's Foreign Secretary Arthur James Balfour and the historic text from his letter sent to Walter Rothschild, 2nd Baron Rothschild, a leader of the British Jewish community.

The reverse side depicts Brigadier General Shlomo Green, later to become Chief Rabbi of Israel, sounding the shofar at the Western Wall on June 7, 1967. 

The edge is inscribed "AINA 50th Anniversary 1967-2017". 

It is circular shaped measuring 2" in diameter and 3/16" thickness.
