The disks have NOT been tested, so they are being sold AS IS as a collector's item.
There is a very good chance they are all good, but that is not guaranteed.

Sorry I do not know the model of the VGA card these came with.

The envelope they came in has a couple bends in the flap and a tear in the front center - no damage to the disks. (see pictures)
