This item was an innovation in the late 1800's! It is a clip to hold on the stiff cuffs of the era. This particular clip was patented in on Feb. 5 1888 (and is stamped with "Pat. Feb 5, 1888") and is referred to as the Wizard Cuff Clip. There is quite a bit of speculation on the internet that these cuff clips doubled as a way to cheat when playing cards. This device was invented to keep the cuffs in place but may have been used by someone to hid a card up their sleeve!
