I don't remember which case this is. It's likely a Casetronic or Travla.

The rack ears can go on either side of the case. In the pictures, they are attached to the back so that I could have all my networking ports at the front of the rack.

For an IO bracket, search ebay for "travla universal IO" - someone here sells them. That's what I have in there now.

It can fit 2 2.5-inch drives.

There's a riser in there. I believe it's 4x electrical.

I replaced the fans with 3 40mm Noctua's. I only ever kept very low power boards in there, so it was never an issue for me. If you put anything in there that generates heat you might want to consider putting regular, screeching server fans back in there :)
