This is an extremely rare new old stock 1950's Mono-Plak truss rod cover in flat Gold finish.
It came from a lot of NOS items of a music store from the 50s. This is not the modern reissue.

The hole spacing on these is 2.1" and total length is approx. 2.6".
Although it has never been used, it does have some light signs of wear. Some scuffs, small patina marks, very light pitting.
