It is ideal to keep on your person, at home, in your car, or to give to a relative or carer.  We have printed 100s of these cards but take care over each and every one.

You can now also specify which region you would like to appear on the card, for example England, Scotland, NI or Cyrmu.  The default card shows all four (one in each corner of the card)

As shown on our sample card, we can fit a lot of information on the card, despite the relatively small size.  Please be aware that the lettering can be quite small be we will always ensure that it is readable and contact you prior to printing if there are any issues.

It may be necessary to abbreviate some medications or medical conditions in order to fit all the information on to the card.

Please ensure that all medications and medical conditions are spelt correctly as we can not be held responsible for any errors in this regard.

These cards do not constitute proof of identity and should only be used in circumstances where, for example, you need to provide the emergency medical information to a health care professional.
