Very good condition.  The glo worm head is in excellent condition, there is barely any wear on it.  Some excess glue on the right side of the plug base.  There is a tiny crack by the right plug prong, doesn't affect anything, it's solid.  There is also a tiny light crack by the excess glue, barely can see it in the picture.
