Does the door require assembly? And how many packages will I receive?

Yes, you need to assemble them, but it is easy to do so. Customers will receive two packages, one package contains 3 door panels, the other package contains the barn door track set, handle and door frame
What is the minimum ceiling clearance required?

Minimum ceiling clearance required 90" Height ( 84" door height + 6" clearance for the track kit above the door)
Is header board included?

The header board is not included.
