Sue's "Orriginal" Patterns #122.  It was originally multi-size, but it's cut to size 10, the smallest size..  IAppears to be from the  1970's or 1980's.  I found it in a box of square dance patterns, so I am guessing that is what it was used for, but it doesn't state that it's a square dance pattern. I have been in the pattern reselling business for about 22 years, and I have never previously seen or heard of Sue's Original Patterns.  

Finished sailor blouse would have puffed sleeves and v-neckline.
