Most likely from a small Atlas, Dunlap or Craftsman lathe. Found loose in a crate of used lathe parts.

Gib screw divots are located approx.:
Fist divot centered from the left end: 9/16"
Center to center: 1-5/16"
Centered from the right end: 5/16"
Used with signs of wear. Items have been left "as-found".
