Brief information based on the entries in the document:
- The pilot was born in 1944. In 1964, he graduated from the Sasovo Civil Aviation Flight School in Sasovo, Ryazan Region, RSFSR (USSR). In 1965, he was appointed as a Yak-12 pilot. In 1966, he was assigned as a co-pilot of the Antonov An-2. Since 1974 he was appointed commander of the An-2 aircraft.
- He worked for the Chernihiv United Aviation Detachment, which at that time was based at the Kolychivka Civil Airport, located near the village of Kolychivka, close to Chernihiv, Ukrainian SSR. The airport operated An-2, An-24 aircraft. It carried out scheduled passenger flights to Kyiv, Moscow, and other cities, as well as air ambulance, agricultural, and other civil aviation operations.
Document condition and nuances::
The document shows signs of age and use, including surface wear, tears, and stains on some pages. One original page, numbered 27/28, is missing. This was the page intended for recording the pilot's disciplinary violations. It has been replaced with a newly inserted Record of Violations page (with red stripe, see photos # 16,17). It is possible that the pilot wished to conceal previous entries, or that the recorded violations had been officially cancelled or removed. Please take this detail into consideration.
The document was acquired in this condition. I have not altered, restored, or replaced any pages myself. Apart from the missing original page described above, all other pages are present. Thank you!
