The item was purchased from a local Hot Topic in 2015.  The item is not graded.  The additional weight of the included figure base is why the shipping costs more than usual.  The box is unopened but has flaws.  Both windows have scratches, and the front window has scuffs along the right edge.  The front panel has scuffs below the bottom edge of the window.  The left panel has a group of scratches to the right of the middle of the back edge.  The rear panel has scuffs above the Star Wars logo.  The bottom panel has a broken line vertical impression which goes from the front edge to the back edge.  The price sticker is still attached to the bottom panel.
