This model is from the Star Trek: Enterprise episode "First Flight."
In the episode 'First Flight' we learned that before Captain Archer took command of the Enterprise, he was a test pilot who was working on the NX project. The NX-Alpha was an experimental vessel that was the first Earth ship to fly faster the Warp 2.5. The ship was designed by John Eaves who gave it echoes of Zefram Chochrane's ship The Phoenix.
