This is for a JC Penney branded 4 inch CRT TV Dual Power Monitor Receiver, model PP-125 Vintage. Even though it says JC Penney, it was actually made by Samsung. It does power on and work, you can see in the pictures, but it is missing its antenna. I think the hole in the picture with the green arrow is where the antenna should be.
