This key tag has the logo of the Grand Army of the Republic.  It is hand-stamped by the owner.  The name is 

M. W. Miller, 426 No. Avon St.  Miller is a veteran of the Civil War.  I was unable to find any information on him or attribute it to a city or state.

The piece is nickel,25mm with a blank reverse.
