Cut off the center pin, then bend the two legs at a right angle so they point behind the transistor. Clean the heat-sinks off with a paper towel or shop rag to remove any old heat-sink compound. You don't need new heat-sink compound with the silicone insulator. For each new output, put the new silicone insulator on the back. Then mount the new transistor on the heat-sink so the two legs go through the two holes and the metal tab of the transistor lines up with the top hole where the old transistor was bolted down. Once you have installed all of these and the pins are installed in the sockets, you can bolt them down to the heat-sink/amp board using the old transistor mounting bolts. The trick here is that the metal tab is the transistor's collector, the same connection as the third pin that you cut off, so by bolting it down you are electrically connecting it to the board. There's a picture in this listing to show how they mount. The transistors in the pic are different, but it's the same concept. The heat-sink and other transistors are not part of this listing.
