Tony Bennett signed I Left My Heart in San Francisco 12" LP in very good condition.  Tony signed the LP in NYC on 12/3/14 with blue sharpie marker.  Very little wear on LP; also there is the number "5" written in what appears to be ink on the back of the LP.  See photo gallery.
