1 handwritten, signed, Fern Formica bank check. Personal Fern Formica bank account check number 877, dated Jan. 29, 1985, made out to "Magi's Puttery" in the amount of $75.47. The check is hand signed Fern Formica and completely handwritten in blue ink. Bank stamps on front and reverse. Fern Formica passed away Jan 23, 1995.
