This is a fantastic card from the 1994 (94) Parkhurst 1964-1965 (64/65) Tall Boys Greats insert set.  This is card number 5 of the set (it is actually unnumbered) and features a pop-out photo of Montreal Canadien Hall of Fame player, Maurice Richard.  These cards were inserted in 1 of every 126 packs.

As a long time collector, it is time for me to thin out my collection.

This card is in great shape for a card that is over 30 years old.  There are no noticeable marks or dings, the pop-out section has not been punctured, and overall I would say that the card is in near-mint condition, going straight from pack to pages, but I am not a professional dealer so please feel free to ask any questions you have.  The condition you see in the scans is of the actual card.  The card has been kept in a dry and smoke-free environment.
