I installed an LED kit in my controller so I have no use for these OEM buttons.
Please check carefully, this set has the ribbon coming from the right side of the USB port (see photos).
Version D, Suitable for JDS-040 / JDM-040.
Comes with the:

Directional buttons.
Left stick / L3 button.
Right stick / R3 button.
and inner touch pad button.
Also 3 tools to help take away the controller.
They were used for about a week and then replaced, check photos for condition.
